Well done series - takes the time to go deep into what happened, and the chain of decisions/pressures that led to it. Incredibly sad of course...But the saddest part is that this was not enough to avoid the Columbia disaster...

Quite enjoyed this documentary series! Didn’t know much about the story! Perfect length of 4x1 hour episodes and well documented from all angles - the family, the decision makers, the investigators, etc! It’s a prime example of how the wrong incentives and organizational/political pressure coupled with group-think mentality can lead to devastating outcomes! I think the lesson to learn here for us watching is that when something doesn’t feel quite right (in social or work circumstances) we need to speak up and question the status quo!
